ForgeVita LLC operates strictly as a technology and intake routing platform. We do not practice medicine or pharmacy. All clinical evaluations, provider disclosures, professional fee authorizations, and prescriptions are managed independently within the secure partner workflow.
Service Description
ForgeVita LLC is a non-clinical technology and management services company that operates a digital health platform connecting patients with independent licensed medical providers and pharmacies. ForgeVita does not practice medicine or pharmacy. Clinical services are provided exclusively by independent licensed medical practices and providers.
By accessing or using the ForgeVita platform, you agree to be bound by these Terms of Service, our Privacy Policy, and any additional terms presented during the intake process. If you do not agree to these Terms, do not use this platform.
Patient Eligibility
You must be 18 years of age or older to use this platform. You must be physically located in a state where the requested program is configured for routing at the time of care. You must not be subject to any legal prohibition on receiving the services requested.
Patients with certain substance-related felony convictions may be ineligible for controlled-substance programs. Eligibility determinations for clinical services are made solely by independent licensed providers and are subject to applicable state and federal law. ForgeVita does not make clinical eligibility determinations.
By submitting your intake, you represent and warrant that you meet the eligibility requirements above and that all information you provide is truthful and accurate.
Medical Disclaimer
ForgeVita does not provide medical advice, diagnosis, or treatment. Nothing on this platform constitutes the practice of medicine, nursing, pharmacy, or any other licensed healthcare profession.
Treatment is never guaranteed. Independent providers have full clinical discretion to evaluate, approve, decline, or modify treatment recommendations based on their professional judgment and applicable law. A submitted intake does not guarantee a prescription, diagnosis, or any specific clinical outcome.
This platform is not for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use this platform to seek emergency care.
Charges & Billing
The initial assessment submitted through the ForgeVita platform has no cost to complete. If your request moves forward through the intake process, the secure clinical partner workflow will disclose the treating provider entity, applicable consents, professional fees, pharmacy charges, lab fees, and any ForgeVita platform fees before clinical evaluation or billing begins.
Charges vary by program, state, and treatment type. You will not be billed without your explicit authorization. All fees disclosed within the clinical partner workflow are the responsibility of the patient or applicable insurance payor.
ForgeVita platform fees, where applicable, are separate from independent provider fees and pharmacy charges. Detailed billing information is provided during the intake process prior to any charge being incurred.
Cancellation Policy
ForgeVita's platform fee, where applicable, is a technology access fee separate from any clinical subscription maintained by independent licensed providers or pharmacy partners. You may cancel the ForgeVita technology platform fee at any time. Email admin@forgevita.com or use the cancellation link in any ForgeVita billing email. Provider and pharmacy cancellation policies are disclosed within the secure clinical workflow and are handled directly with those parties. No cancellation fees and no long-term platform contracts apply to the ForgeVita technology platform fee.
ForgeVita does not issue refunds for completed clinical evaluations performed by independent providers. If you have a billing dispute related to clinical services, contact the treating provider or pharmacy directly using the contact information provided in your secure partner portal.
Medical Records
Clinical records generated during your care are owned by the treating provider or medical practice, not by ForgeVita. ForgeVita LLC is a non-clinical technology company and is not a medical records custodian.
To request your clinical records, contact your treating provider directly. Provider contact information is available in your secure partner portal. For platform account data requests, email admin@forgevita.com.

Limitation of Liability
To the maximum extent permitted by applicable law, ForgeVita LLC, its officers, directors, employees, agents, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of, or inability to use, the platform or any services routed through it.
ForgeVita's total aggregate liability to you for any claim arising out of or relating to these Terms or the platform is limited to the amounts paid by you to ForgeVita in the twelve (12) months immediately preceding the claim giving rise to liability.
This limitation does not apply to claims arising from gross negligence or willful misconduct by ForgeVita, or to claims arising from medical malpractice by independent providers, which are governed solely by the applicable provider-patient relationship and applicable law.
Assumption of Risk
You acknowledge that telehealth services may have inherent limitations compared to in-person care. A remote clinical evaluation may not capture all clinically relevant information that an in-person examination would.
You acknowledge that technology failures, including network interruptions, platform outages, and third-party service disruptions, may occur and may affect the delivery of services. You acknowledge that independent providers may decline to provide services, request additional information, or require in-person evaluation based on their professional judgment.
By using this platform, you voluntarily assume the risks inherent in remote clinical evaluation and digital health platform services as described in these Terms and the Telehealth Consent.
Prohibited Uses
You agree not to use the ForgeVita platform for any of the following:
- Any unlawful purpose or in violation of applicable federal, state, or local law;
- Submitting fraudulent, false, or misleading medical or identity information;
- Reselling, redistributing, or diverting prescription medications obtained through independent providers;
- Harassing, threatening, or abusing platform users, staff, or independent providers;
- Interfering with or disrupting platform systems, servers, or networks;
- Attempting to gain unauthorized access to any portion of the platform or related systems;
- Collecting or harvesting personal information of other users without authorization.
Violation of these prohibited uses may result in immediate termination of your account and may be reported to appropriate law enforcement authorities.
Intellectual Property
All platform content, design, technology, trademarks, service marks, trade names, and intellectual property are the exclusive property of ForgeVita LLC or its licensors. All rights are reserved.
You are granted a limited, non-exclusive, non-transferable, revocable license to access and use the platform solely for your personal, non-commercial purposes in accordance with these Terms. This license does not permit reproduction, modification, distribution, or commercial exploitation of any platform content or technology.
Unauthorized use of ForgeVita's intellectual property is strictly prohibited and may result in legal action.
Governing Law
These Terms of Service are governed by and construed in accordance with the laws of the State of Wyoming, without regard to its conflict of law principles.
Any dispute, claim, or controversy arising out of or relating to these Terms or the use of the ForgeVita platform shall be resolved through binding individual arbitration administered by the American Arbitration Association (AAA) in accordance with its Consumer Arbitration Rules, except for claims that may be properly brought in small claims court.
By agreeing to these Terms, you waive the right to participate in a class action lawsuit or class-wide arbitration. You and ForgeVita each retain the right to seek injunctive or other equitable relief in a court of competent jurisdiction to prevent actual or threatened infringement or misappropriation of intellectual property rights.
