This Consumer Health Data Privacy Policy describes how ForgeVita LLC collects, uses, shares, and protects "Consumer Health Data" under the Washington My Health My Data Act (RCW 19.373), Nevada SB 370, and similar state consumer-health-data laws. This policy applies in addition to (and not in place of) ForgeVita's general Privacy Policy.
This policy does not apply to Protected Health Information governed by HIPAA. PHI is handled by the Affiliated Providers (and our clinical platform partner where applicable) under each entity's separate Notice of Privacy Practices.
ForgeVita does not sell Consumer Health Data. ForgeVita does not share Consumer Health Data for targeted advertising except with your affirmative consent. You may request access, deletion, or withdrawal of consent at any time by contacting privacy@forgevita.com.
