ForgeVita LLC operates strictly as a technology and intake routing platform. We do not practice medicine or pharmacy. All clinical evaluations, provider disclosures, professional fee authorizations, and prescriptions are managed independently within the secure partner workflow.
Who Owns Your Records
Clinical records generated during your care (including provider notes, prescriptions, lab results, and clinical communications) are owned by the treating provider or medical practice, not by ForgeVita LLC.
ForgeVita is a non-clinical technology and management services company. We are not a medical records custodian. The independent licensed providers and medical practices that deliver care through the platform bear responsibility for the creation, maintenance, and release of clinical records under applicable law.
Your rights with respect to your clinical records are governed by the applicable policies of the treating provider or medical practice, and by applicable state and federal law including, where applicable, the Health Insurance Portability and Accountability Act (HIPAA).
What ForgeVita Accesses
ForgeVita accesses platform data (including intake form submissions, account information, and technical usage data) for the following limited purposes:
- Platform operations and routing of intake requests to independent licensed providers;
- Customer support and account management;
- Security monitoring and fraud prevention;
- Administrative services as a management services organization (MSO) to independent medical practices;
- Legal compliance and regulatory obligations.
ForgeVita does not access or store detailed clinical records (including provider-patient communications, clinical notes, prescription details, or diagnostic results) beyond what is strictly required for the purposes listed above. Clinical workflow data is maintained within the secure partner infrastructure.
Requesting Your Records
To request your records, use the appropriate pathway depending on the type of information you need:
Contact your treating provider directly. Provider contact information and secure messaging are available in your secure partner portal. Your provider is the custodian of all clinical records from your care encounters.
Email admin@forgevita.com with the subject line "Account Data Request" and your full name and email address on file. We will respond within thirty (30) days.
Certain data may be subject to legal retention requirements that prevent immediate deletion even upon request. We will notify you if a retention obligation applies to your request.
Records Retention
Clinical records are retained by the treating provider or medical practice for the period required by applicable law, which varies by state and provider type.
ForgeVita retains platform data (including intake submissions and account information) in accordance with its Privacy Policy. Platform data is retained while your account is active and for a reasonable period thereafter as required for legal compliance, dispute resolution, and business operations.
Analytics and technical data is retained for up to twenty-four (24) months. Payment records are retained as required by applicable financial regulations. After applicable retention periods expire, data is deleted or anonymized.
Provider Information
Independent licensed providers operating within the ForgeVita platform maintain their own patient record policies, intake consent documentation, and applicable HIPAA or state privacy notices. These documents are presented to you within the secure clinical partner workflow prior to the commencement of any clinical evaluation.
ForgeVita is not the treating provider or medical-record custodian. Depending on the workflow, ForgeVita may act as a non-clinical service provider or business associate to independent medical practices.
