Independent Provider Network Structure
ForgeVita is a non-clinical MSO technology company. All clinical decisions are made by independent licensed medical practices and providers — not by ForgeVita.
ForgeVita LLC is a non-clinical technology and management services organization (MSO). Clinical services — including medical evaluation, diagnosis, prescribing, and treatment — are provided by independent licensed medical practices and providers. ForgeVita does not practice medicine or pharmacy.
How Provider Relationships Work
- Independent licensed providers make all clinical decisions
- Providers retain full discretion to evaluate, approve, decline, or modify treatment recommendations
- ForgeVita routes intake requests; it does not direct clinical judgment
- The treating provider entity is disclosed in the secure clinical partner workflow before clinical evaluation or billing begins
- Provider and pharmacy fees are separate from any ForgeVita platform fees
Depending on state, program, and partner workflow, your reviewing clinician may be a physician or another appropriately licensed clinician.
Provider Disclosure & Operating Models
- Provider entity disclosed in the secure clinical partner workflow before evaluation
- Consents, payees, and charges shown before clinical evaluation or billing
- Platform routes assessment; all clinical decisions made by independent provider
- Currently waitlist-only pending controlled-substance workflow validation
- When available: may require labs, identity verification, video evaluation, or in-person follow-up per state law and DEA Schedule III requirements

Provider Disclosure Before Care
The treating provider entity's name, contact information, applicable consents, payees, and final charges are all disclosed in the secure clinical partner workflow before clinical evaluation or professional billing can begin. ForgeVita does not authorize clinical care or billing.
Medical Records
Clinical records are owned by the treating provider or medical practice. ForgeVita is not a medical records custodian. See our Medical Records Policy.
