ForgeVita LLC is a non-clinical technology and management services organization (MSO). Clinical services — including medical evaluation, diagnosis, prescribing, and treatment — are provided by independent licensed medical practices and providers. ForgeVita does not practice medicine or pharmacy.
What ForgeVita Is — And Is Not

Technology Routing Platform
ForgeVita builds and operates the intake assessment forms, encrypted routing channels, and partner-workflow integrations that connect patients with independent licensed providers. We are a software and management services layer — not a care delivery organization.
A Medical Practice, Prescriber, or Pharmacy
ForgeVita employs no physicians, nurse practitioners, physician assistants, or pharmacists in a clinical capacity. We issue no prescriptions, make no diagnoses, and exercise no clinical judgment. All medical decisions are made solely by independent licensed providers.
A Transparent Layer
Before care begins, patients see the charges, the provider entity responsible for their clinical care, and all relevant consents. ForgeVita is structured so patients always know what they are agreeing to and who is delivering their clinical services.
The Clinical Fulfillment Chain
Clinical evaluation, treatment decisions, and prescription fulfillment are outside ForgeVita's scope. Independent licensed providers have full and sole discretion over clinical appropriateness. Independent licensed pharmacies manage dispensing and shipping. ForgeVita does not control or override either party.
How The Model Works
Four distinct stages — each handled by the appropriate party. ForgeVita's role ends at stage two.
Assessment
Patient completes a health assessment through ForgeVita's intake platform. Responses are routed through encrypted channels to the appropriate clinical partner workflow.
ForgeVita LayerSecure Handoff
ForgeVita's secure clinical partner workflow activates, transmitting intake data to the independent licensed practice managing the clinical relationship for that state and program.
ForgeVita LayerIndependent Clinical Review
A licensed independent provider reviews the intake, evaluates clinical appropriateness, and exercises sole medical judgment — approving, modifying, or declining the case entirely. ForgeVita has no role here.
Independent ProviderPharmacy Fulfillment
If a prescription is issued, a licensed independent pharmacy reviews, dispenses, and ships the medication directly to the patient. ForgeVita is not the dispensing entity and exercises no pharmaceutical oversight.

Privacy By Design
Intake data is routed through encrypted channels and transmitted only to the clinical partner responsible for evaluating that patient. ForgeVita does not sell, rent, or monetize patient health data.

State-by-State Availability
ForgeVita's routing rules are configured on a state-by-state basis to reflect each jurisdiction's telehealth laws, provider licensure, and program-specific regulations. Not every state is enabled for every program. Check the State Availability page to see current routing configuration — and remember that "configured" means routing is enabled, not that treatment is guaranteed.
Compliance-Forward Operations
The MSO model ForgeVita operates under is specifically designed to maintain structural separation between technology services and clinical practice. This is not a legal workaround — it is the architecture required for a technology platform to responsibly support clinical workflows without practicing medicine.
No Clinical Override
Independent licensed providers have full discretion to decline any case for any clinical reason. ForgeVita does not set clinical protocols, influence prescribing decisions, or create volume-based incentives for providers. A provider declining a case is the system working correctly — not a failure state.
