ForgeVita LLC operates strictly as a technology and intake routing platform. We do not practice medicine or pharmacy. All clinical evaluations, provider disclosures, professional fee authorizations, and prescriptions are managed independently within the secure partner workflow.
What Telehealth Means
Telehealth involves the delivery of healthcare services using electronic communications, information technology, and asynchronous or synchronous interactions between a patient and an independent licensed provider. This may include online intake forms, secure messaging, telephone consultation, and video conferencing.
Your physical state location is verified at the start of intake to confirm program routing availability and applicable provider licensure. Your location may be required to be reconfirmed at the time of clinical evaluation by the independent provider. Providing a false location is a violation of these Terms and may constitute fraud.
Telehealth services provided through the ForgeVita platform are delivered exclusively by independent licensed medical providers. ForgeVita does not employ or supervise these providers and does not direct clinical decision-making.
Potential Benefits
Telehealth services may offer the following potential benefits compared to traditional in-person care:
- Convenience of accessing care from your location without travel;
- Reduced time between initial request and provider evaluation in many cases;
- Faster routing to an independent licensed provider configured for your state and requested program;
- The ability to receive follow-up care and provider communication without in-person visits in many cases;
- Access to programs that may not be available from local in-person providers in your area.
These are potential benefits and are not guaranteed for any individual patient or care request. Actual outcomes depend on your clinical circumstances and the independent provider's evaluation.
Limitations & Risks
Telehealth may not provide the same depth of information available from an in-person clinical evaluation. The independent provider conducting your evaluation may not be able to perform a physical examination, observe clinical signs in person, or access all relevant diagnostic information.
As a result, the treating provider may require additional steps before proceeding with care, including:
- Laboratory testing or bloodwork at a local facility;
- Medical imaging or diagnostic studies;
- A synchronous (real-time) video or phone consultation;
- An in-person evaluation with a local provider.
Technology failures, including network interruptions, platform outages, device failures, and third-party service disruptions, may occur and may delay or interrupt the delivery of services. There is no guarantee of diagnosis, prescription, or treatment availability through this platform.
Privacy & Security
ForgeVita uses commercially reasonable technical safeguards to protect data in transit between you and the platform, including encryption of data transmitted over public networks.
The secure clinical partner workflow has its own privacy protections, data security measures, and applicable legal obligations. Communications within the clinical workflow are governed by the treating provider's applicable policies and applicable law.
No method of electronic transmission is 100% secure. You acknowledge and accept this limitation by using telehealth services. For more information, see our Privacy Policy.

Provider Discretion
Independent licensed providers operating within the ForgeVita platform retain full clinical discretion at all times. A provider may, based on their professional judgment and applicable law:
- Decline to provide telehealth services for any clinically appropriate reason;
- Request additional medical information, records, or laboratory results before proceeding;
- Require a synchronous consultation or in-person evaluation;
- Prescribe a different treatment than requested, or decline to prescribe;
- Refer you to a different level of care.
All clinical decisions are made solely by independent licensed providers. ForgeVita does not influence, override, or direct provider clinical judgment. Submission of an intake request does not create a provider-patient relationship with ForgeVita and does not obligate any provider to treat you.
Emergency Notice
This platform is NOT for medical emergencies. If you are experiencing chest pain, difficulty breathing, stroke symptoms, suicidal thoughts or intent to harm yourself or others, a severe allergic reaction, or any other life-threatening condition — call 911 immediately or go to the nearest emergency room. Do not use this platform to seek emergency care.
If you are in crisis and need immediate mental health support, contact the 988 Suicide and Crisis Lifeline by calling or texting 988.
ForgeVita and independent providers operating through this platform are not equipped to respond to medical emergencies. Response times through this platform are not suitable for time-sensitive or emergency clinical situations.
Communications & Records
Electronic records generated during your telehealth encounter will be retained by the treating provider or medical practice per applicable law and their operational needs. These records belong to the treating provider, not to ForgeVita.
ForgeVita retains platform data — including your intake submission and account information — per its Privacy Policy. For clinical record requests, contact your treating provider directly through the secure partner portal.
Communications between you and an independent provider within the clinical workflow are not accessible to ForgeVita except as required for platform operations, security, or administrative services as a management services organization.
Patient Acknowledgments
By proceeding with ForgeVita's intake process, you acknowledge and confirm the following:
- You have read and understood the potential benefits and limitations of telehealth services described in this consent;
- You understand that remote care may have limitations compared to an in-person clinical evaluation;
- You understand that independent licensed providers have full clinical discretion and may decline to provide services;
- You understand that this platform is not an emergency service and is not a substitute for emergency medical care;
- You understand that technology failures may occur and may affect service delivery;
- You understand that your information may be shared with independent licensed providers and pharmacies as necessary to coordinate care through the platform;
- You are providing your voluntary, informed consent to receive telehealth services through independent licensed providers accessible via the ForgeVita platform.
This consent is effective upon your submission of the intake form and remains in effect for subsequent care requests unless you withdraw it in writing. To withdraw consent, contact admin@forgevita.com. Withdrawal of consent will not affect care already received.
